It falls apart on mobile due to lack of horizontal space.
Ay suggestions for an alternative solution for mobile?
How about an accordion layout? Just make sure that only one sheet can be open and put the submit button in the active sheet, as suggested in this article: http://baymard.com/blog/accordion-and-tab-design
